Annual Report Form for Non-Parochial Priests

SUBJECT:                  Annual Report
 
It’s that time of year again when I need to ask you to complete an Annual Report. In general you need to submit an annual report in the attached format if you:
 
§ do not serve in a parish or mission on a paid basis, or 
§ are 72 years of age or older, or
§ are retired from active service, or
§ reside in or are officiating in another diocese, or
§ are canonically resident in another diocese. 
 
I include with this memorandum the text of some relevant Canons and procedures for licensure in the Diocese of Central Florida. 
 
You will note that we ask you on the Report Form to verify that you have been trained in some very specific courses:
1. “Safeguarding God’s Children” – which training may be completed online by making arrangements through your local congregation “Local Administrator” or through Marilyn Lang in my office.
2. “Safeguarding God’s People” – both units (Prevention of Exploitation and Prevention of Harassment) have been offered twice and are scheduled twice more in the next few months. A flyer with upcoming training dates is attached.
You must certify that you have completed these specific trainings OR that you plan to complete them OR indicate why you cannot do so.
 
I want to make this as simple as possible for you so I ask you to print out and complete this form and return it to me by April 1, 2010. I would appreciate it if you could get this to me sooner because we both know how easily these things can get misplaced if set aside. If you prefer to receive a printed copy of this memo and enclosures, please let me know and I will send this to you via U.S. Mail.

Thank you for your cooperation. Signing the form means that you certify the information is correct and (where appropriate) are applying for a renewal of your license to function as a priest in the Diocese of Central Florida and/or wish to remain in your present tenure.
